The current Atrial Fibrilliation (AF) SLA can be found below, if you want to start providing this service please read this and complete the sign-up form in Appendix 2 however please email the form to peter.prokopa@nhs.net. The completed Signature form (Appendix 3) still needs to be emailed to Andy Pickard a.pickard@nhs.net who will authorise the PharmOutcomes module to be made live for you.
