New service: Shropshire Wellbeing and Navigation Network (SWANN)

Do you work with adults (over 18) who live in Shropshire who may need help with supported health discharge, community-led mental health outreach, or wellbeing and independence support? If so, I’d like to introduce you to the Shropshire Wellbeing and Navigation Network (SWANN), a new service commissioned by Shropshire Council and the Integrated Care Board (ICB).

SWANN is underpinned by the new Preventative Services Contract, which you may already be familiar with and may have contributed to during the commissioning process.

The service is delivered by our partners British Red Cross, Shropshire Mental Health Support, Community Resource, and Live Well Shropshire (part of ICE Creates). It’s really simple to refer local residents, ensuring that they get the support they need to stay well, remain independent and feel connected within their own communities.

Who’s it for?

Shropshire Wellbeing and Navigation Network supports adults (18+) who may be:

  • Finding it harder to manage at home
  • Experiencing low-level mental health challenges
  • Socially isolated or lacking confidence
  • Recovering after illness or hospital discharge
  • Living with long-term conditions
  • Unpaid carers who need additional support

 What are the benefits?

  • Early support before problems escalate, reducing the need for crisis or statutory services
  • Help to stay well and manage everyday life
  • Opportunities to build confidence, reduce isolation, and connect with local communities
  • Practical and emotional support, including assistance after hospital discharge, wellbeing activities, and peer support for mental health
  • Support for unpaid carers and those living with long-term health conditions

 How does it work?

Support is delivered by a range of local organisations and includes:

  • Supported hospital discharge (provided by British Red Cross)
  • Community-led mental health outreach (provided by Shropshire Mental Health Support & Community Resource)
  • Wellbeing and independence support, including group activities and befriending (provided by Live Well Shropshire)

Referrals are managed through a central point, coordinated by Shropshire Council. Our teams match the adults referred to the support that’s best for them. Referrals can be made by staff in health services, adult social care, primary care/social prescribing, housing, and voluntary/community organisations – with the person’s consent.

How do I refer a resident?

Just call Customer Services on 0345 678 9012 and select option 2.
