Workplace CVD Health Checks EOI – FINAL VERSION

Telford & Wrekin Council are seeking Expressions of Interest to implement a CVD Health Check pilot in workplace settings across Telford and Wrekin up until 31 March 2025.

Expressions of Interest to be received by 24 October 2024.

The pilot aims to gather evidence on the feasibility and impact of workplace CVD checks and spot the early signs of heart disease.

They are seeking Expressions of Interest from Community Pharmacy Team(s) who can work with the Council’s Health Improvement Team to undertake CVD checks, record on PharmOutcomes and provide results and information to individuals to reduce their risk of having a heart attack, stroke or developing type 2 diabetes. There will be a requirement for onward signposting/referral to appropriate organisations, particularly the council’s Healthy Lifestyles/Stop Smoking Service, Community Pharmacy and General Practice.

The pilot looks to deliver 2500 CVD checks in total. The main partner identified to support this delivery and work towards achieving this target is Community Pharmacy. A total of £48,000 has been ringfenced for Community Pharmacy to express their interest in delivering this element of the pilot.

The total value of this contract for the delivery of the CVD health checks is £48,000. Telford & Wrekin Council are seeking EOIs from either an individual community pharmacy or two community pharmacy teams working jointly to deliver part of the contract.
